A Florence Historical Foundation Support form is available in two formats.  You can use the form below or view and print the form in PDF file format.  The easiest way is to a PDF reader (such as Adobe Acrobat Reader) to print the form.  If you do not have a PDF reader, you may click on the graphic link (below) to get Adobe Acrobat Reader.  It is free and allows viewing of many documents found on the Internet.

Click on the support form below to view the form by itself.  Your monitor's resolution may affect the quality viewed, however, it should print correctly.  To print the form depends on the maker and version of browser.  Most browsers allow you to print the current webpage.  Internet Explorer 5 (and later) allows you to select the File | Print option.  If you get a warning about margins, print anyway.  The form is made 1/4" less than a standard 8 1/2 by 11 page.  Some printers will not be able to print to the extreme edges.  The only thing you will miss is the "fold" lines.  If you cannot print the form at all, you may request a form be sent to you.  Request the form be sent by e-mail or regular mail.
